If you mean "click to talk", where you are on a web page, and click a URL to
create a phone call, this is described in the 3pcc draft:

If you mean something like the Nextel Direct service, where you can push a
button and then directly speak to a group of people, this is doable with
existing SIP (in a few different ways, in fact), but I am not aware of any
IETF documents that have yet published flows for this service.
