Its 487 to unify treatment with the case where the UAC sends CANCEL upon 
expiration, which will result in a 487. It really doesn't matter.

Sure, although its usefulness is limited, since they are usually 
answered immediately.

No. You only send CANCEL after a 1xx has been received. If you never get 
any response at all, its likely there is some kind of server failure and 
so a CANCEL would not likely accomplish anything anyway.

UAC can elect to CANCEL if it receives no *final* response after the 
expiration; that is, after all, what it is interested in.
