You may use the disabled media position (or slot) not only to reactivate the
disabled media stream but also to "add a new media". The main reason to
re-using  the disabled media posion is that, the media lines are ever
growing if you add a new media line and keep carrying the old disabled media
lines. Hence siply re-use the position that has been disabled by setting the
port number to zero.

Yes (if you want the new media to be received on a different port).

No. The following para limns it clearly. You can change the port number of a
stream by just changing the port number and leaving the rest untouched. No
modification to this section is needed, IMO.
