After MSN Messenger is set up as Eran has indicated, there is one
problem that you may encounter(at least from what I have seen from
experimentation).
After registration is successful, you will be signed into MSN Messenger
and everything will appear as normal except that all of your contacts
may appear as Not Online (RED). This is because MSN Messenger sends a
SUBSCRIBE for each of your contacts to the specified server. If this
server can forward the SUBSCRIBE to the appropriate contact or forward
the SUBSCRIBE to a presence server, a NOTIFY reply should be generated.
When your MSN Messenger RTC Client receives the NOTIFY indicating that
the contact is online, the Contact will appear as Online (GREEN) and
everything should work from that point forward. If the NOTIFY reply is
not received, the contact will appear as Not Online (RED). Incoming
calls and messages can still be received in either case. 

BTW, MSN Messenger 4.6.0077 is now available.

I faced this problem also.

You need a local Proxy server (even a redirect server is not enough) In
tools->Options->Accounts Choose Communications service And uncheck all
.NET related stuff.

In Communications service account pick your sign-in name.
Click advanced... and enter your Proxy server address.

Once you do sign-in the messenger will send REGISTER to the Proxy.

If 200 Ok will be returned you can make calls to your mates (if the
proxy know how to resolve their address) and upon INVITE will forward
the INVITE to them. If you have a redirect server that will return 3xx,
The messanger does not support that and the call will be terminated.

I am talking of  messenger version 4.6 (4.6.0076) The latest I think.
