Based on bis09, a server
(line3644) " ....If the host portion of the "sent-by" differs from the packet source 
address, the server MUST add a "received" parameter to that Via header field.(the 
received source ip)...."

Question: under what circumstances will a server received a SIP message having 
"sent-by" not the sender ip (besides domain name) ?

Also, (line 3679) states that a server shall sent a response for UDP to the "received" 
parameter, and the port of the "sent-by" parameter. 

Question: Are we assuming that the intermediate host will open a receiving port as the 
host stated in the "sent-by" parameter ? Who enforce the rule ?
