Yes, assuming that your B2B does not also host registrations, it would
modify the SIP message accordingly and forward it to the next hop (which
may be another proxy, or the intended registrar)

Dear all:
When a B2BUA is used as an ALG (to deal the firewall/NAT), how would it
handle REGISTER? Would it "proxy" the REGISTER for the sending UA?
What I am thinking is to add a "Via" header and then forward it to the
proxy server. When the 200OK returns, the B2BUA will just relay the
response back to the sending UA. Does this make sense?
