I would suggest '503' as is used for transaction errors (RFC3261 section 8.1.3.1) and sometimes for transport errors (16.9)

RFC 3262 Section 3 says  "If a reliable provisional response is
retransmitted for 64*T1 seconds without reception of a coresponding PRACK,
the UAS SHOULD reject the original rquest with a 5xx response."

Which 5xx response is appropriate?

None of the 5xx error responses defined in RFC 3261 seem appropriate:
500 Server Internal Error
501 Not Implemented
502 Bad Gateway
503 Service Unavailable
504 Server Time-out
505 Version Not Supported
513 Message Too Large

504 seems like the best candidate, but the description seems vague in
Section 21.5.5.
The server did not receive a timely response from an external server
it accessed in attempting to process the request...
