There doesn't seem to be any rule that a UA is required to accept an
out-of-dialog REFER.  And it seems like there could be privacy/security
reasons to not accept them.  So your first two examples seem OK to me,
even if they are inconvenient.  The last two examples seem to be
outright bugs.
