   deployment or use case where this header is used?

As you note, RFC 3261 says that Accept-Language is used to express
preferences regarding text phrases in SIP messages, and has nothing to
do with the human language used in the audio stream.

I don't know whether there are any SIP applications that also use it
to specify audio language.
