ACK has never been optional. Every INVITE's response gets an ACK.
In general, you will render media when you have media. Once you send the answer (which I assume you are doing in the 200 in your question), you MUST be prepared to receive media and you SHOULD render it. If you don't get the ACK, you SHOULD terminate the session with a BYE (see the top of page 86 of RFC3261).
