But it doesn't make sense. If the UAS accepts the INVITE but includes 
"Require: XXX" in a provisional response, what does it mean? what is supposed 
the UAC must do? cancel the current INVITE and send a new one with such 
"Require"?

No, for that the UAS should reject the first INVITE with 421 code ("Extension 
Required"). 

IMHO such "Require: XXX" in a provisional response is just wrong and the UAC 
should ignore it or cancel the request (because the UAS is buggy).
