I think we can send 400 response to such request. 
RFC never mandates any such thing but it is wise to do so.

But I am not able to understand the use case for sending multipl To
headers.

Max Forward:
If Proxy is in between then Proxy MUST insert the same with default
value of 70 and in case the Request is directly reach UAS, then UAS can
reject the Request, but as uage of Max-Froward is to detect the looping
and block the same, henec in absence of that the call may loop if UAS
has to extedn the call again to proxy, in that case Proxy can insert
this header.

Max forward is not of much use if the request has reached its
destination. We can still process the request.

Here I am with a doubt that, Wat the UAS is going to do when it receives
multiple "To" or "From" headers in the initial INVITE???

Do we need to send 400 response for that??? RFC 3261 doesn't say to send
400 response. And is it ryt way of giving the preference to the first
"To" or "From" headers if multiple headers present??

And also watz the response of UAS if no Max-Forward header in the
INVITE??.
If proxy is in between, proxy will had Max-Forward and sends. If no
proxy and INVITE directly receives to end UAS, watz the response of
UAS??
