Many thanks for the replies. Valdemar, i have seen such call flows but when 
using a PBX such as 3CX for example, the REFER with replaces is sent
to the PBX which then sends an INVITE without a replaces header to the 
respective UA. Those diagrams don't really deal with a 'real life' situation 
where INVITEs are going via a PBX so i think that is why i haven't been able to 
see these in practice.

Any comments welcome.
