As an fyi, the UAC/UAS labels within the figure are not accurate since
requests are sent is both directions.

User Agent Client and User Agent Server are defined within RFC 3261
section 6.

recommendations.  If I recall correctly, the RFC doesn't provide a
recommendation which would explicitly cover your example (i.e. sending
UPDATE during INVITE/re-INVITE when session-expires has not yet been
activated).

Concerning the example, the refresher value must be uac (i.e. see RFC 4028
section 9).  Negotiated refresher becomes callee.
