There are a (growing) number of ways to transmit DTMF information. A particular UA that is interested in receiving DTMF needs nail down a way of doing so. And a UA that is to send DTMF needs to nail down the way(s?) it will be sending it.

After the initial invite there needs to be some way for the recipient to decide if a way has been agreed upon. If not, then it still may have the option of sending an explicit subscription. But it needs to know if that is required. If nobody every explicitly confirms that they agree to send DTMF in some agreed upon way then things are messed up.
