I squeezed in an update before the IETF72 cut-off. The changelog:
* Added text in the introduction as well as in sections 4 and 5.3
to address WGLC comments.
* Added Subscription-State field in example flows
* Fixed figure numbering mismatch

(Note that there is a regression since 02: a few typos that were fixed
aren't any more. Even though I was using SVN...)
