Summary: pcapplay - cancel and clean up media threads

Initial Comment:
When using PCAPPLAY with a long PCAP file and a scenario that is shorter than 
the PCAP file, the media thread is left running (and sending media) after the 
call ends.

The same occurs if you play multiple PCAP files inside a scenario; if your 
timing is not quite right, you can end up with multiple media threads for the 
same call.

The attached patch address this is in the following manner:

1. send_packets() is updated to handle being pthread_cancel()'d by way of 
pthread cleanup functions.
2. call::~call() is updated to cancel the media thread if one was created
3. call::executeAction() is update to cancel an outstanding media thread before 
starting a new one
