I assume one can generate nattraversalrules.xml using the nat
traversal page of sipxconfig and save (export) it to the file system -
which ought to ask the supervisor to fire up the affected services (
which in this case would be sipxrelay and SipXProxy ).

If this is not happening, there is potentially a dependency that was
not declared somewhere.
