Andrei,
For what it's worth, I have seen this type of behavior when there is an issue 
with the phone sharing an IP network with the phone system, and the two have 
different gateways, or their VLAN does not directly connect or route to each 
other.
I would also verify the config or the server and the phone to ensure they are 
both using the correct gateway(s). When the UA and the server are on the same 
network with different gateways all calls could get stuck in CDR's and none 
complete properly.
Feel free to ignore me though.

In looking at the logs, the whole system is really not healthy. There 
are a number of transport errors in the Proxy and I can't really tell 
much without debugging turned on in the Proxy to see what sort of 
messages are seen and/or generated. 
Could you please reproduce with debugging turned on.
