I must admit I've never used a satellite connection.  Do we expect SIP
to be run over such connections?

More to the point, how do we handle situations where most destinations
are likely to be less than 100 msec away, but some are over 800 msec
away?  It seems like the choice is to have a low initial timeout, but a
fairly high ultimate timeout.
