I think the Polycom is reporting MOH dialogs in error.  It only happens
in the requisite full-state NOTIFY following a sipXrls SUBSCRIBE (or
re-SUBSCRIBE.)

My hope is that the Polycom fix will simply be to omit MOH dialogs from
full-state NOTIFYs.
