Once a subscription is established, the contact that the server provides
is "anchored", it contains the IP address of the server with which the
subscription is established.  So all refreshes of the subscription will
be at the same server.

My suspicion is that the phone has lost its subscription and established
another one, which happened to be routed to the other server.  The two
subscriptions will look very similar, but the Call-Ids will be
different.  But especially with Polycom phones, you have to examine
Call-Ids carefully to know if they are the same or not, as only a few
characters differ between the different Call-Ids generated by a single
phone.
