You may be able to follow what sipXbridge does in those cases.  If I understand 
what you are looking for, you are using FS they way we currently use 
sipXbridge...and I believe sipXbridge is currently doing the right thing with 
respect to attended transfer REFER/INVITE w Replaces.  At a minimum, just try 
an a consultative transfer from a polycom phone dialing out through sipXbridge 
(which can just be back-to-back with another sipXecs, no need for a real ITSP).

Examples can be very enlightening, but please don't fall into the trap of 
assuming the examples are all you need.  The RFC's cover more than the 
examples, and many assumptions from just looking at examples turn out to be 
incomplete.  So, look at the examples (here 
[http://www.tech-invite.com/Ti-sip-service-5.html] for instance), and the 
traces, but also read the RFC to try to see why.
