These changes are necessary, of course, but I would like to discuss
another change which might help in cases where sipXecs crashes or is
otherwise not able to store the IMDB.  I think it would be safe, when
SipPersistentSubscriptionMgr loads the internal subscription table from
the IMDB, to add some arbitrary amount to the restored CSeq and XML
version.  In most cases, this would ensure that the next generated
NOTIFY would be acceptable, even if subscription.xml was behind reality.
