The calls can be linked by using a new field called "reference". It may
contain 0 or more comma separated "linked" callids along with the
relationship in the form of <callid>;rel=<relationship>.
Possible relationships that we use in 4.2 are: xfer, refer and chain.

Please note that the linking occurs from the last to first legs of the
call.
