Turn on debugging for the Proxy and for CDR and after the failed
scenarios, take a snapshot and include CDR records as well as logs.

The calls can be linked by using a new field called "reference". It may
contain 0 or more comma separated "linked" callids along with the
relationship in the form of <callid>;rel=<relationship>.
Possible relationships that we use in 4.2 are: xfer, refer and chain.
The meaning of these relationships are described in the Internet Draft
for "The References Header for SIP"
http://tools.ietf.org/html/draft-worley-references-04 .

Please note that the linking occurs from the last to first legs of the
call.
