Well, I just fixed a MOH related issue a related phone called Nortel
SMC 3456  ( A.K.A. bria ). Your issue is possibly related.

Counterpath does things differently when it comes to holds and
transfers and it does not support native MOH. Anyway, you can
experiment with the latest development snapshot and let me know how it
goes.. Please see here:
