Unknown (to me).  But as setup/authentication is the very expensive part,  
and that only happens once per host at boot time, one can safely assume  
it's less than doing setup/auth on every call.

Another way to look at it:
VPN:
   setup/auth/encryption X (number of hosts in a cluster)

Call by Call TLS:
   (setup/auth/encryption) X (number of calls)


And yes, the VPN is only for sipX-sipX calls, and can also be used for  
securing non-SIP interprocess communications.  It does not prevent the  
need for talking SSIP to external entities.
