I was looking at a trace in XX-7348 and discovered that if the proxy receives 486 and 404 responses on two forks, it returns 404 upstream. It seems to me that returning the 486 upstream is probably more useful, as sometimes 404 responses are generated by forks due to "features not implemented" (such as a user that does not have voicemail).
