We have no links between the incoming and outgoing subscriptions, and we
have no mechanism for reporting incoming subscription failures
(phone->service) to the service.  The current design has the application
publishing status, and the SubscribeServer takes care of maintaining
subscriptions and sending NOTIFYs as appropriate.
