I've been looking at the sipxprovision and REST code to figure out how
to auto-provision my phones completely but while I have been able to add
the phones to SipXconfig I don't know what kind of credentials to give
to them so they can make calls immediately as e.g. Polycom phones can.
Where do I get those credentials?

Actually, phones don't need credentials to make calls, as long as (1) the 
destination of the call is one that sipX is not configured to require 
permissions for, and (2) the phone does not use as its From URI a SIP URI that 
sipX has credentials for.  But until the phone is provisioned with a proper SIP 
identity, you haven't given it permission to call destinations that require 
permissions.  And until you provision the phone with a proper SIP identity, it 
can't be claiming to be a user with credentials.
