Specific steps I can follow to test this. You'll need some way to have phones from outside the local domain, I'm using an unmanaged gateway for that with a regular ISDN phone attached. For local phones I'd recommend Polycom phones since their templates are already adjusted. Project the phone configurations to add the alert info handling to their config.
Calling the local Polycom phone with a local phone should show no difference with the default settings. In the proxy settings you can then enable internal call handling, after that you should see an Alert-Info field attached to the header reading <http://internal.call>;info=alert-internal;x-line-id=0. Calling the same Polycom phone with the external phone should by default attach an Alert-Info reading <http://external.call>;info=alert-external;x-line-id=0 and trigger a different ring.
