Excellent!  That seems to have been it.  Specifically, I had been getting lazy 
setting up new systems and had been leaving the hostname as 
localhost.localdomain.  You made me realize "maybe that is bad" so I went ahead 
and set a real hostname, ran sipxecs-setup and then restarted sipxconfig and 
everything came up!

Thanks again, I was really in the weeds!

I think I've seen this obscure error before, check that your
/etc/hosts file looks something like

192.168.1.2 swift.hubler.us swift
127.0.0.1   localhost localhost.localdomain localhost4 localhost4.localdomain4
::1         localhost localhost.localdomain localhost6 localhost6.localdomain6

where first line is most important and in this exact order
{LAN IP}  {FQDN} {HOST}
