Excellent! That seems to have been it. Specifically, I had been getting lazy setting up new systems and had been leaving the hostname as localhost.localdomain. You made me realize "maybe that is bad" so I went ahead and set a real hostname, ran sipxecs-setup and then restarted sipxconfig and everything came up!
