Jim,

Can you cat the content of /etc/sipxpbx/sipxhomer.ini?  Also please try 
to set the log level of homer to debug and see if there are any logs 
pertaining to this error.

Mine looks like this.

sqa-control-port=5240
sqa-control-address=192.168.1.10
log-level=6
db-url=DATABASE=homer_db;SERVER=localhost;UID=root;DRIVER=MySQL;READONLY=0;
