Yes, there were lingering registrations, so I created a new user and only 
registered it from the Internet, then added the alias I needed. It goes 
straight to voicemail, even though the phone is registered. If I VPN in, 
register the phone, both MWI and DID work, but if I am remote, it does not.

In the registration it shows 

<sip:201@<remote_user_public_ip:30578;rinstance=68807c1be45ee19c;x-sipX-privcontact=<remote_user_private_ip%3A30578>

So yes, it is registered.

Have you confirmed that the remote phones are really registered?

Do the addresses in those registrations match the public addresses of
the remote phones?
