Did you get 4.0 via an upgrade??  Reason for this question is that I've
seen this error when there is a permission problem, say files are not
owned by sipxchange:sipxchange and sipxconfig cannot perform the update.

For starters, check
/var/sipxdata/mediaserver/data/mailstore/<extension> and replace
<extension> with the extension for the user that is having problems. I
believe all the files in there should have sipxchange:sipxchange as
owner.

Try to reproduce it again. Once you do go to Diagnostics->Snapshot. Get
a
snapshot. Open an issue and attach the snapshot.
