Yesterday I had call quality issues that I determined was caused by my ISP
by running some bandwidth tests (we're using an ITSP for inbound and
outbound calls). Here's one of the sites I like to use to test my bandwidth:

The bandwidth test showed that the upload speed and TCP pause values were
very poor, which is inline with the quality of the call; the incoming audio
quality was fine, but the outgoing audio quality was terrible. In this case,
I feel that the bandwidth test results are fairly accurate. However, I have
found in the past that they are not always accurate and my ISP never trusts
the results when I contact them about poor quality.

I would like to know if there are any troubleshooting steps that I can take
to truly identify (with little or no doubt) where the cause of the problem
is? I'd also like to get some feedback about what others do when they run
into call quality issues and what tools you use. Also, are there any tools
that can notify you about call quality issues, rather than relying on users
telling you that there is an issue?
