I'm not sure I follow you. If you are multihomed to the internet, it is certainly possible to have sipx sitting behind it. There is an excellent document on the new wiki that was written and discusses how to use DNS preferences to give desired results.
Now as far as your router is concerned, I don't know enough of what you are trying to do. I certainly wouldn't put sipx "on" the router. Most router packages have dynamic dns agents that can change outside based bind files based on their Ip address. What it sounds like you might be after is a router package to take multiple connections, do dynamic dns and publicsh the results properly.
