I'm not sure of the details, but it means that the phone that placed the
call did not have proper permission to call the destination phone.
Check that the permissions on your dialing plans are set the way you
want them.  In particular, if the destination phone is an analog phone,
does the call go through a gateway to get to the analog phone?  Check
that the permissions are set correctly as to who can make calls to the
gateway, and that the originating phone is properly configured (and
registered) with a user that is permitted to call through that gateway.
