In this test there is no provider, I'm outside the firewall on my MAC and 
register my normal office extension in 'Telephone' (a mac soft phone based on 
pjsua/pjsip) then call '0'. I get the AA and can interact with her via RFC 2833 
style inband DTMF (so the RTSP path is open/working both ways to the machine 
running sipXecs) for about 30-45 seconds and then 'Telephone' reports a 
timeout. If I transfer to a Polycom extension I get no talk path and a timeout.

This is almost certainly because I haven't done the 'remote worker' setup, I 
was about to try that then recalled seeing warnings against enabling the nat 
traversal settings on the list a while back.

I'll try combining the two setup guides from the old wiki and see if I can make 
something that works both ways -- and report back.
