Upgraded the firmware on the firewall in front of the sipx server. Gremlins all gone now. calls fwd into and out of the network like they should and on voip.ms, both (either) static and/or user/auth works for both inbound and outbound.

can even accept inbound from one itsp, have it answered by the extensions private AA and have it FWD out a different ITSP via the 'dial 0'

packet traces on the sipx server vs packet traces using tshark on the outside of the firewall show that despite told NOT to, the firewall was mucking with the insides of the packets before sending them, along with trashing the source ports.
