Here are a few questions:

1- (silly but gotta ask) Is there any chance that 210 and 211 have the
same IP address?
2- Is there anything different about this site vs. the 34 others?  Is
the combination of [phone model+fw revision, NAT model+fw revision] at
the faulty site being successfully used in any other site.
3- Can you cut&paste the registration contacts for 210 and 211 in
sipXconfig->Diagnostics->Registrations so that I can look at it?
4- What is the model of the NAT used are the faulty site?

My best guess is that the NAT at the faulty site has difficulty
disambiguating between two endpoints on the private network that both
use port 5060 a source port and both send to the same destination
IP:port.  To [dis]prove that theory, is it possible for you to
reconfigure *one* of the phones to use a non-standard source SIP port
(6060 for example), reboot *both* phones, let them register and try to
make outbound calls.
