The phones have v 3.1.3 firmware, so I won't address them other than to say you should remove one MOH source so there is only ONE MOH source for calls. Remove the MOH uri from the phone config via sipxconfig and resend the profile to one phone. Try placing your call on hold from that phone with MOH enabled at sipxbridge.
