After detailed debugging I was eventually able to determine
that it was a problem with the caller ID.  After resolving
that, I no longer got the "forbidden", but I continued to
have other connection and routing issues.

Finally I set up an account at Vitelity and within 10
minutes I had a functioning phone system.  I think the
problem may just be the way some of these ITSP provide SIP
services.
