Sending calls to the operator was the key.

Calls were answered properly so I know it is something with
client. That I can figure out later.

For the record, the DID sent from vitelity is 10 digits. And
setting up a sub account (on Vitelity) that points to your
IP address and port 5080 appears to be necessary for
Vitelity to send calls to sipxbridge.

Also settings that are working for me with Vitelity:

Address: 64.2.142.93
ITSP Server Address: 64.2.142.93
everything else default as of 4.2
