Sending calls to the operator was the key. Calls were answered properly so I know it is something with client. That I can figure out later. For the record, the DID sent from vitelity is 10 digits. And setting up a sub account (on Vitelity) that points to your IP address and port 5080 appears to be necessary for Vitelity to send calls to sipxbridge. Thanks for your help.
