All of this is in the wiki. The phone sends its local ip address and
registers under the public address. Look at the registration page in sipx.
It is so the phone can talk to sipx while traversing its local firewall.

Look at the registration of the phone in sipx. Does the phone show "two" ip
addresses in the registration?

Make sure the phone is configured properly (tolpology settings), nat=use
loac address, stun=discover, xtunnels=never.
