I think MOH needs to be on by default in sipxbridge. Some ITSP's have a MOH
setting option. On trunks this should be "off" at the provider.

Is MOH "configured" (moh uri) in the polycom? It should not matter as a
trunk call if MOH is enabled at sipxbridge.

I think if MOH is off at sipxbridge, the ITSP might not like the moh style
(ietf draft) that the polycom is sending, and can;t ACK it, so it drops the
call...

Keepalive and RTCP works fine, but since there is a dialogue event to
establish media "and_if_the_itsp_does_not_like_the_moh_style_or_moh_uri"),
it will not work.

I think you might need to use MOH 0.0.0.0 on the phones (polycom) and off at
sipxbridge and see if that works. I'm assuming the itsp would be compatible
with the MOH uri (old style) of 0.0.0.0 directly with the phone.

(my opinion, turn on moh at sipxbridge and make sure it is off at the itsp).
