The problem you are experiencing is "latency". This is typically an issue,
so you want to connect to the PSTN in a way that does not make you have to
"hop" all the way back. If the latency is because of a saturation at a point
closest to your traveler, almost nothing will help the call quality, no
matter what application you use.

I would agree that getting around it with another provider or connection
that doesn't "hop too many places" to get you on/off the pstn is the way to
go.

I might suggest first to try gmail with the little outbound calls for free
ap embedded in the chat window, because it is perhaps the simplest thing to
deploy. A headset (or speaker/mic) with a web browser to make those outbound
calls. Trying that if the quality is "ok" might be the "simplest" but not
the best...

I'm not a skype user or fan, as I find the software to be "virus like" in
the way it handles resources on a PC, but sometimes you have to do whatever
you need to get over a hump...
