5060, udp/tcp if you support inbound sip: url calls and/or remote users.
5080/udp/tcp for ITSP's
30000-31000 UDP.

note: turn off all sip helpers, alg, etc.
you MUST USE 1:1 STATIC NATTING or you get one sided voice calls, if anything.

there are a LOT of FAQ's on this, so, wherever you got 30000-31000 tcp, ignore it, blacklist it, don't listen to anything it says.
