The cancel is sent becuase two lines up the ITSP sends you a "BYE" which sipx 
sends down to the phone to cancel the call.

Try getting a sipxtrace of a good call and then a sipxtrace of a bad call.  It 
will be easier to pinpoint the difference.

where it goes south... 
    Why is the cancel sent to from the sipx server to the final transfer
