I had the same issue when I first setup up an external
voicemail server. In my case the calls were originating from
a Cisco call manager.

The fact that calls are being hit by the voicemail IVR I
think is because the voicemail cannot identify which mailbox
the message is intended for i.e. it cannot read the DNIS.  A
fix was applied in my case which made it into 4.2.1. what
specific version are you using?

A temporary fix could be to divert the calls to mailbox
using the 8 (by default) prefix to the extension.  However,
you may need to insert and strip some steering codes to get
it to work.
